Recommend empiric coverage of Enterococcus with ampicillin (200mg/kg/d IV div q6h) or amoxicillin-clavulanate (> 1 month old) if:

• <1 month old

• urinary catheter

• instrumentation of urinary tract

• genitourinary abnormality
