- Line must be removed if any of:

  • tunnel infection
  • failure to respond within 72 hours (persistent positive daily blood cultures)
  • fungal (with a percutaneous CVL).

- Line removal recommended if at all possible.

Amphotericin B 0.6-1mg/kg IV daily 14 days after first negative BC

Fluconazole 12mg/kg/d IV/PO div q12h 14 days after first negative BC
